Co-Filing Forms
Summary
This table identifies frequent groupings of forms when submitting packages to E-Filing. It should be noted that a Form 1 accompanies ALL filings whether it is an initial filing or a subsequent filing. This is not intended to be an all-encompassing list, but identifies the standard groupings of common submissions. Unless specifically stated otherwise, the forms included in each package are mandatory.
