Paper forms: A corporation key is required for all paper transactions and must be submitted with your form. You will need to verify your identity online.

Form 15 - Articles of Revival

Please note a few things before completing the form:

  • You must include a cover letter with your request to revive a corporation. See Instructions for completing Form 15 – Articles of Revival for more details on what to include in the cover letter.
  • You must include a valid Nuans name search report for the corporate name. The Nuans report is not required if the corporation has been dissolved for less than 2 years or had a numbered name.
  • Corporations Canada can refuse to issue a Certificate of Revival if there is a valid reason to do so. Fees are not reimbursed if the request is not approved or is withdrawn by the applicant.

    There are a number of circumstances where Corporations Canada will refuse to issue a Certificate of Revival, including (but not limited to):

    • The corporation has been dissolved for a while (for example, more than 2 years) and has not been carrying on business. Usually in this case, a new corporation should be created instead of reviving the dissolved one.
    • The corporation was voluntarily dissolved, so there should be no remaining assets or liabilities, and it should not be carrying on business. Only in extraordinary circumstances would Corporations Canada consider a revival.
    • The corporation was dissolved for not paying the incorporation fee.
  • The Express service is not available at this time.
